UpCurrentFM does not accept health insurance. Patients will be charged via the credit card, debit card, FSA card, or HSA card on file at the conclusion of every visit. UpCurrentFM can provide you with a Superbill, upon request, which you can submit to your insurance company for potential reimbursement.

Visits with UpCurrentFM Functional Medicine Provider, Zoie Phillips, MSPA, PA-C, IFMCP, AFMC:

  • New Patient Visit (90 minutes)- $900 $600

    • Included in this fee is:

      • 90+ minutes of your provider’s time preparing for your visit

      • 90 minute visit with your provider

      • 30+ minutes of your provider’s time finalizing your treatment plan

      • Individualized treatment plan (available within 48 hours after your visit)

  • Comprehensive Lab Review Visit (90 minutes)- $900 $600

    • Included in this fee is:

      • 90+ minutes of your provider’s time reviewing your comprehensive labs and preparing for your visit

      • 90 minute visit with your provider

      • 30+ minutes of your provider’s time finalizing your treatment/supplement plan

      • Individualized treatment/supplement plan (available within 48 hours after your visit)

  • Follow Up Visit (45 minutes)- $450 $300

    • Included in this fee is:

      • 45+ minutes of your provider’s time preparing for your visit

      • 45 minute visit with your provider

      • 30+ minutes of your provider’s time finalizing your treatment plan

      • Individualized treatment plan (available within 48 hours after your visit)

Visits with UpCurrentFM Functional Nutritionist/Health Coach, Melissa Reade, CDN, CNS, NB-HBC:

  • Established UpCurrentFM patients who have attended at least a New Patient Visit with Zoie may schedule Nutrition/Health Coaching visits with with Melissa:

    • Step 1

      • Attend Nutrition/Health Coaching New Patient Visit (90 minutes)- $350

    • Step 2

      • After the Nutrition/Health Coaching New Patient Visit, Melissa will recommend either package to be used over a span of 6 months:

        • Package 1: Nutrition/Health Coaching Follow Up Visits (45 minutes) x4- $700 $650

        • Package 2: Nutrition/Health Coaching Follow Up Visits (45 minutes) x6- $1050 $975

    • Step 3

      • After a Nutrition/Health Coaching package (1 or 2) has been completed, patients can:

        • Purchase another Nutrition/Health Coaching package (1 or 2)

        • Purchase Nutrition/Health Coaching Follow Up Visits (45 minutes) à la carte- $175
